FIB-4 and Elastography in Fatty Liver: When Is Further Testing Needed?
Seeing fatty liver on ultrasound does not by itself show how much scarring (stiffness) the liver has. In people with metabolic risk factors, advanced fibrosis risk is assessed with blood-based scores such as FIB-4 and, when needed, elastography.
Are fatty liver and fibrosis the same thing?
Steatosis means fat build-up in the liver; fibrosis means scar tissue formation. Excess weight, type 2 diabetes and other metabolic problems matter in the assessment. Normal liver enzymes do not reliably rule out advanced disease.
What is FIB-4 used for?
It is a risk-assessment tool that uses age, AST, ALT and platelet count. It is not a cancer test and does not diagnose cirrhosis on its own. Age and acute illness can affect its interpretation; calculating it online and deciding alone is not appropriate.
When is elastography considered?
The 2024 EASL–EASD–EASO guideline recommends methods that measure liver stiffness, where needed, after blood-based assessment in the appropriate risk group. If the risk is high or the result unclear, a gastroenterology/hepatology assessment is planned.
Preparing for the treatment discussion
Medications, alcohol use, diabetes and weight history are reviewed. A plan for nutrition, physical activity and weight management is tailored to the person. “Liver detox” products do not replace this assessment. When obesity treatment is needed it is assessed separately; not every fatty liver requires surgery.
