Mirizzi Syndrome: Diagnosis and Surgical Approach
Mirizzi syndrome is obstruction caused by external compression of the adjacent common bile duct by a stone impacted in the gallbladder neck or cystic duct. Accurate diagnosis is critical for surgical strategy.
Symptoms
- Jaundice
- Right-upper-quadrant pain
- Fever
- Nausea
Diagnosis
Ultrasound is the first step. MRCP evaluates the cystic duct, common bile duct and any fistula; ERCP may also be used when needed.
Classification
The Csendes classification ranges from pure external compression to advanced forms with a fistula between the gallbladder and the common bile duct.
Treatment
Treatment is surgical, but the approach depends on the Mirizzi type. Early types may be completed laparoscopically, while advanced types may require bile-duct reconstruction.
